Session times and cost
Our evidence-based prostate cancer group exercise program operates in five convenient locations in the Perth metropolitan area.
Each session costs just $12, with rebates available through Private Health Funds and DVA patients may be entitled to further concessions.
Extended Care Plans (ECP) are available with Medicare funding upon referral by your GP for these prostate cancer exercises.

Complete the online PROST! Registration form.
To attend the evidence based group exercise sessions, each member must undertake a pre-exercise assessment with our qualified Exercise Physiologist. This ensures you are covered by insurance during the sessions.
Download the Adult Pre-Exercise Form. There is a $56.00 fee for the assessment. You may be eligible for a Federal Government assistance by downloading the Chronic Disease Management Plan letter to be provided to your GP for a referral. Once signed by your GP, take it to the assessment and provide it to the Exercise Physiologist who will complete the form and collect the payment.
Arrange an appointment with the Exercise Physiologist (see details above) at your nominated venue. They will have access to your completed membership registration form to undertake a physical assessment.
Please take the pre-exercise screening form and if applicable, The Chronic Disease Management Plan Referral with you to the appointment.
